Senior Care Management Services
The Senior Care Management Program provides links and referrals to frail, mostly low-income seniors over 60 who reside in the central, southern, and southwestern sections of Morris County and who want to remain in their own homes and live independently. Care managers carefully assess the needs of the seniors and their caregivers and create individualized plans of care. There are many seniors in Morris County that desire to remain in their own homes but need the additional support and reinforcement that our home-based services can provide. We also to provide support, respite and counseling to our senior clients’ caregivers.
Cornerstone links Senior Care Management Program members to a comprehensive range of services, including: home health aides, home delivered meals, money management services, services for the blind, grocery shopping, Friendly Visitation, housecleaning, in-home counseling services, Cornerstone Adult Day Services, Advanced Practice Nursing, in-home doctor’s visits, in-home podiatrist’s visits, and transportation. We also provide low-cost home maintenance project referrals, home safety checks, free income tax preparation, caregiver counseling and respite services, and legal aid and elder law attorney referrals.
